Discharge Planning
The discharge planning process is focused on the continuing care needs of the patient and the family. It is important for you to understand your medical condition and begin on admission or soon thereafter to participate and plan for continuing care prior to discharge. This process combines the expert knowledge and experience of all your care team who will help you and your family identify health and wellness needs after your hospital stay.
Day of Discharge
When your doctor determines that you no longer need specialized inpatient hospital services, he or she will issue discharge instructions and orders. Your nurse will review the instructions with you so you understand your continuing care needs. Arrangements will also be made to assist you to your vehicle or your ride home.
Case Management
Sacred Heart Hospital offers supportive counseling and discharge planning services to patients and families. These services, provided by Case Managers and Social Workers, may include arranging services such as home health care, nursing home placement, rehabilitation placement and other community aid to meet your continuing care needs after discharge. Home Health & Hospice Services
Your care team will help you look at all the options available for assistance at home following discharge. To assure that you receive the assistance that you need, supportive help, home care nursing, therapy, or hospice care can be arranged.
